David Harriman, PharmDClinical consulting & expert witness services

The work

A clear account of the evidence.
A careful explanation of its limits.

  1. Medical records review

Orders, administration records, laboratory results, and clinical notes are connected into a chronology. Findings identify the supporting record, unresolved discrepancies, and the information still needed.

  2. Hospital systems analysis

Review examines how work moved through the hospital: who had which information, what checks were available, and how concerns were escalated. Harriman brings experience conducting root cause analyses and medication error reviews.

  3. Expert opinions & testimony

Plain-language explanations of pharmacy practice and medication issues, with opinions tied to the evidence. Written reports and testimony are separately scoped to the questions and Harriman’s expertise.

Applications include medication injury and malpractice, sentinel events, and clinical questions in fraud, whistleblower, and patient safety retaliation matters. Legal conclusions remain with counsel.
